These questionnaires are to help the doctor better assess your overall Neurological impairment/Neurotransmitter imbalance. Please read the question, quickly answer true or false, and move on to the next question. Do not think about each question in detail, since that will create an inaccurate test result. When finished, simply add up the scores at the end of each section and bring only the scores to the office for review.

Secure Your Initial Consultation At Kemp Functional Neurology, we provide a deep-dive assessment to uncover the root causes of your neurological concerns. To begin this process, we require a $50 appointment holding fee.
Please note that this $50 payment is applied directly toward your total initial consultation fee. Once you have submitted your intake forms and the holding fee has been processed, a member of our team will contact you to finalize your appointment details and confirm your visit.
